Make a Referral
To join Renaissance Foundation, young people must:
Be aged between 12-16
Be based in Greater London with the ability to access Renaissance Foundation Youth Hub (in the City of London)
Have a long-term health condition*
… and/or be a young carer
Have good cognitive ability to take part in activities
If you are a teacher, HCP, GP, young carer support worker, social worker or another professional who knows an eligible young person you can refer them to Renaissance Foundation by clicking on the links below.
If for any reason you are unable to complete a referral form please email outreach@renaissance-foundation.com to set up a call with our Outreach team to discuss your referral over the phone.

Refer a Young Patient
*A young patient is someone undergoing regular medical treatment for a chronic; lifelong or life-limiting condition. Conditions include asthma, Type 1 Diabetes, Sickle Cell Disease and Cystic Fibrosis.
Refer a Young Carer
A young person who cares, unpaid, for a family member who has any type of physical or mental illness, physical and/or mental disability or misuses substances.
Self-Referral
Refer yourself directly to Renaissance Foundation if you think you are either a young carer or a young patient and if you are aged 12-16.
How are referrals assessed?
Make a referral through the RF website
Referral should be completed by a healthcare professional or a member of school staff (it can also be a parent in a certain cases)

Referral is assessed by our team
The RF team decide if we are best placed to support the young person and their needs.
Onboarding session at our new Hub
Young person attends one of our onboarding sessions. Once complete, a parent/guardian signs a consent form and their young person is fully enrolled onto the RF 3-stage programme!

It is completely free to join the programme and all the activities we provide are free of any charge to young people.

16, we can accept 17 on a case by case basis.

Sessions take place at the RF hub in Aldgate or visits to inspiring locations.

No, but the individual must be well and confident enough to independently travel to programme activities.

Yes, multiple family members are welcome to participate in the programme.
