Refer a Young Hospital Patient

If you are a teacher, HCP, GP, young carer support worker, social worker or another professional who knows a young carer or patient between the ages of 12 -16 you can refer them to RF. Simply click the Refer a Young Patient button

If for any reason you are unable to complete a referral form please email outreach@renaissance-foundation.com to set up a call with our Outreach team to discuss your referral over the phone.
